Entry Form

Pals for paws are sponsoring a photo contest for the pets of Kokomo. If you have a unique or pretty pet, enter your pet for a chance for a prize package filled with valuable pet supplies and gifts.

Categories include:

• Cutest Dog.
• Cutest Kitty.
• Most unique pet.
• Pets do the darnedest things (This would include your pet doing silly or crazy things)

Entry deadline is March 24thth 2007

Entry fee is $10.00 per category. Entry packets can be picked up in this store or at the Pals store in the mall.

Please return entries to Missy Dennison @ P. O. Box 1034 Kokomo IN 46903 or drop them off at the Pals for Paws store in the Kokomo Mall any Saturday before or on the deadline. Entries will be up for viewing and voting on March 31st and April 7th. 2007 from 12p-5pm. The pet photo with the most amount of support will win.
Paws of wisdom and rules

• The first place winner in each category wins a prize. Prizes can be viewed in the Pals for Paws store in the mall.
   
• You are permitted to enter your pet in more than one category but will require a picture for each separate category. This will also require a separate entry fee and photo.
   
• You are permitted to vote for your own pet. Get your family and friends involved. Patrons in the mall will also be stopping by to view the pictures and vote. Winners will be notified via contact number after the end of the contest.
   
• Attach your picture to the entry form and return it back to Pals for Paws at the address above or in the Buddy Center.
   
• At the end of the contest on April 7th 2007 at 5 pm, you will be able to pick up your pictures. A self addressed stamped envelope can be included with your picture and we will try and have your picture mailed back if you are unable to pick it up. Pictures can also be picked up on the fallowing Saturday April 14th 2007. Any pictures left after April 21st 2007 after 5 pm may be discarded.
   
• Pals for paws Inc and its volunteers are not responsible for lost or damaged pictures. So don’t give us the only copy if it is of great value.
   
• Please be sure to put your pets name and owners name on the back of the picture so the right pic goes with the right entry.
  

Proceeds go to Pals for Paws and the homeless pets we support. We are a not-for-profit organization that helps homeless animals find forever homes. We rely strictly on donations and fundraisers. Please help support the pets.
